Body Art
Health Card Requirements
Per the Regulations Governing the Sanitation and Safety of Tattoo Establishments all tattoo operators must possess a valid body art health card.
In order to obtain
a health card, the operator or technician must meet the following requirements:
- Free of communicable
diseases
- A minimum of 18 years of age
- Have a minimum
of six months experience or training
- Obtained a score of at
least 80 percent on a written examination
- Tested for
tuberculosis by skin test or chest x-ray
- Immunized against
hepatitis A and B
- Possess current certification by the American
Red Cross in Preventing Disease Transmission
New and Late Renewal Health Card Instructions
Application Instructions
- Complete the application in person at the Henderson Public Health Center Monday through Friday between 8 a.m. and 4 p.m. (map & directions)
- An appointment to take the written exam will be scheduled upon receipt of the application and exam fee.
- Submit the following with the application:
- Written proof from a previous employer of at least six months experience or training.
- Written proof must contain a date, be on letterhead (if possible), specify month/year dates of experience and have the signature both printed and signed.
- Written proof from a Clark County Health Permitted Establishment stating the applicant will be working in his or her establishment.
- Written exam fee of $118 (non-refundable) payable by cash, money order, Visa or Mastercard.

Immunizations and Training
Within 30 days of passing the written exam, all applicants must begin their Hepatitis B vaccination series and take the required American Red Cross training.
Early Renewal Health Card Instructions
Health cards may be renewed within 60 days of the expiration date.
Application instructions
- Complete the application in person at the Henderson Public Health Center at 520 E. Lake Mead, Henderson, Nevada. (map & directions)
- Applications are accepted Monday, Tuesday and Friday between 8 a.m. and 4 p.m. and Wednesday between 8 a.m. and 2:30 p.m. except on holidays and days when the TB skin test cannot be read in 48 to 72 hours.
- Records will be reviewed for complete hepatitis B series and valid Red Cross training card.
- If employment has changed, provide written proof from a Clark County Health Permitted Establishment stating the applicant is be working in his or her establishment.
