Body Art or Microblading Apprentice Card

Applicants who do not have six months of experience must apprentice for a minimum of six months in an establishment permitted by the Health District, working under a mentor who possesses a Body Art Mentor Card. Apprenticeship activities cannot begin until the Body Art Apprentice Card or Microblading Apprentice Card has been issued.


To be eligible for a Body Art Card or Microblading Apprentice Card, the applicant must:

  • Be a minimum of 18 years of age.
  • Have a minimum of six months of experience or training.
  • Possess current certification from a course on bloodborne disease transmission that meets the requirements of the OSHA Bloodborne Pathogens Standard.
  • Possess valid identification that adheres to our ID policy.
  • Print and complete the Apprentice Application and Employment Verification form, with the Mentor Information section completed and signed by the mentor. Bring the form in printed, not electronic, format.
  • Schedule an appointment to take a 30-question, multiple-choice sanitation examination.
  • Obtain a score of at least 80 percent on the exam. Results of the exam will be available immediately. Applicants who do not pass can retake the exam. Prepare for the exam by reviewing the training books and the Regulations Governing the Sanitation and Safety of Body Art Establishments:

After working for at least six months under a Body Art Mentor, the apprentice should:

Cash, debit, Visa, MasterCard, American Express, Discover, Apple Pay, Google Pay, money order, cashier’s checks and business checks are accepted at all locations.
