Our office is located at 2830 Fremont St, Las Vegas, NV 89104. This location is closed on Mondays and cash is not accepted.
The Plan Review program processes new, remodel, and change of ownership health permit applications for food establishments, cosmetic and drug manufacturers, childcare centers, schools, body art facilities, and Institutions. Plans are reviewed to ensure that they are compliant with the applicable Health District regulation.
Applications must be submitted electronically via the EH Portal and will be processed in the order in which they are received.
To apply for a permit, log into the EH Portal, go Home > Create > Create New > Apply for a Permit. Please gather your required documents, including the applicable questionnaire below.
