The Southern Nevada Health District does not issue immunization exemptions. Immunization exemption requests must be completed and submitted directly to the childcare facility, school, or university using official forms provided by the Nevada Division of Public and Behavioral Health.

Medical Exemptions

Students and children with appropriate medical conditions may qualify for a temporary or permanent medical exemption if authorized by their physician. Medical exemption forms must be completed by a licensed healthcare provider and submitted to the school, childcare facility, or university.

Religious Exemptions

Students and/or parents who wish to exempt a child or student from one or more required immunizations due to sincerely held religious beliefs must submit a religious exemption form to the school, childcare facility, or university.

Important Information

• Exemption forms must be the official forms provided by the Nevada Division of Public and Behavioral Health
• Children with immunization exemptions may be excluded from school or childcare during outbreaks of vaccine-preventable diseases if they are not fully immunized
• Parents are still required to provide official immunization records for all vaccines received, even if a partial exemption is requested. This helps schools and facilities determine immunization status during disease outbreaks and protect public health
