Vaccinations
Before your visit, you will receive a text message or email with a link to complete your registration and consent forms. Please complete these ahead of time to help speed up your visit.
Be sure to provide a valid mobile phone number and email address when booking so you can receive important updates and instructions.
Book your Clinic Location
Mandatory back to school vaccinations, as well as recommended vaccinations, are available at these locations. Immunizations by appointment only.
Birth Certificates
Birth certificates will be available on-site at the Southern Nevada Health District Main Facility.
This is a walk-in event—no appointments required.
Vaccine Information Sheets
The Centers for Disease Control and Prevention (CDC) offers a number of vaccine information sheets providing details of the vaccines required to be administered. Download vaccine information sheets on the VIS webpage.
The Health District supports strategies that are designed to strengthen and sustain public health and improve the health of communities through immunization programs. Download the Immunization Program Policy Statement to learn more.
Parental Consent
Non-custodial adults may accompany a child, however, written consent must be provided to the health district from the parent or guardian at the time of service. See the Parental Consent for Immunization of a Minor Policy for more details.
Costs
For those without insurance, a same day self-pay discount can be applied. Vaccine costs vary based on the type of immunization required and if VFC eligible. In addition to the vaccine costs, the health district charges a discounted administration fee of $22 per person for one vaccine and $15 for each additional vaccine.
Payment made be made by Visa, MasterCard, cash, debit card, money order, cashier’s check and most insurance accepted. Not all vaccines are covered by insurance.

Immunization Records
For vaccinations received in Nevada
The Public Access Portal of the Nevada WebIZ program allows parents and legal guardians to print official immunization records for their child(ren) (aged 0 through 17 years). It also allows adult individuals (aged 18 years and over) to print official immunization records for themselves. To download immunization records, visit the Nevada WebIZ Public Access Portal or call 1-775-684-5954.
For vaccinations received outside of Nevada
If you have lived in a state with an immunization information system, you may try to contact that system where you or your child received their last shots to see if they have your records.
