The health district has birth certificates for Clark County births from 1973 to the present.

Personal checks are not accepted as payment for birth and death certificates.
Newborns
It takes a minimum of two weeks for the health district to receive a new birth certificate from the hospital.
Clark County Births Before 1973
Birth certificates for people born in Clark County before 1973 are available at the Nevada State Office of Vital Records at (775) 684-4242.
Records for Births Outside of Clark County
Birth records for people born outside Clark County, but in Nevada, are available at the Nevada State Office of Vital Records at (775) 684-4242.
For records of people born outside the state of Nevada, contact the vital records department in that birth state.

Birth certificates can be requested by one of the following methods:

You can request a birth certificate in person at:

- Mesquite Public Health Center - 830 Hafen Lane
Office hours are 8 a.m. to 4 p.m., Monday through Friday. Cash, money order, Visa or MasterCard are accepted. Photo identification is required.
By Mail
Mail a money order (made payable to SNHD) along with a completed Birth Certificate Application PDF (419 KB/1 page) and a copy of your photo identification* to:

Fax a completed Birth Certificate Application PDF (402 KB/2 pages) along with the completed credit card (Visa or MasterCard only) information and a copy of your photo identification* to (702) 383-4095.
Online
Complete the online birth certificate application (Visa or MasterCard only). A photo ID of the requestor and credit card holder must be uploaded with the form.
*Applications received without photo identification will not be processed. It is your responsibility to ensure a copy of your identification is received either in person, by mail or fax.
