Testing for the Body Art Card program resumed April 6, 2021.
For those who have not already started the application process by submitting a completed employment verification or experience verification form to the Southern Nevada Health District before April 1, 2021, you can start the application process by scheduling an appointment through the online scheduling portal or by calling (702) 759-0595 to take the sanitation exam.
If you already started the application process by submitting an employment verification or experience form to the Health District prior to April 1, 2021, and received a date stamp on your form, then you need to complete the process by scheduling an appointment to take the exam through the online scheduling portal or by calling (702) 759-0595.
When you arrive for your exam appointment, you will need to provide a valid photo ID, proof of bloodborne pathogen training completed within the last year, the stamped verification form (if that applies) and one of the following:
- Applicants for a permanent Body Art Card must provide proof of at least six months experience or training outside Clark County, NV, in the form of a letter from the shop or facility where they worked, or a permit or license issued from the previous jurisdiction in which they operated if they were self-employed.
- New apprentices must provide a completed Employment Verification/Independent Contractor form showing where they will complete their training.
