Who can order a birth certificate?
Birth certificates may only be released to a qualified applicant. Qualified applicants are the parents listed on the certificate, the person whose birth is registered on the certificate, grandparents* or someone with documentation showing entitlement.
*If the father is not listed on the birth certificate, the only person entitled to order a birth certificate is the mother, the person whose birth is registered on the certificate or someone with documentation showing entitlement.

- If you want to change the child’s birth name (for example, change his or her last name to match that of the biological father), you may do so at this time in Section B of the form.
- Both the mother and the biological father must sign the completed form in front of a witness.
