Congenital Syphilis Case Management:

Syphilis during pregnancy can be passed to your baby and cause serious health problems, including stillbirth, bone damage, blindness, or deafness. But the good news is it’s 100% preventable with the right care. The Congenital Syphilis Case Management Program at the Southern Nevada Health District is here to support you during pregnancy and after delivery if you are diagnosed with Syphilis

What is CSCMP?

A client-centered medical case management program for individuals navigating infectious disease in pregnancy. The client is paired with a specially trained nurse case manager and, together, they work to reduce the client’s barriers to care so that the client can achieve optimal health outcomes. Case management services include linkage to syphilis testing and treatment for clients and their partner(s), assistance with health insurance and prenatal care, and referrals to local community resources (diaper banks, car seats, and more.) Your case manager will follow you all throughout your pregnancy to make sure you stay up to date with recommended follow-up to reduce the risk of transmitting syphilis to your pregnancy.

Eligibility criteria:

This program is offered at no cost to our clients and is available to any pregnant or recently postpartum person that is positive for syphilis. To refer someone or yourself to this program, call 702-759-1111.

Complex STI Navigation:

  • Provides navigation services for evaluation and treatment of neurosyphilis and other complex STI’s.
  • Academic Detailing/Education at local hospitals.

Rapid PREVENT:

Status Neutral Approach to Improve HIV Prevention and Health Outcomes

This program provides non-medical case management services while using a whole person approach to elevate HIV prevention and health outcomes among racial and ethnic minorities.

Project SURE:

Serving Unhoused through Resources and Engagement in Care.

In collaboration with the Nevada Homeless Alliance, the Health District’s SHOPP team participates in service fairs for unsheltered, low-income individuals and families that provide immediate access to a full array of services in one place. These events include housing assessment and/or placement, medical/dental/vision services, behavioral health, job readiness, family resources, food resources, legal and court assistance, general community assistance and more.

The SHOPP team serves as a resource for clients accessing care at the Health District. These individuals are connected to the team’s community health workers (CHW) who assist in linking clients to services and needed housing resources through the Nevada Homeless Alliance and its partner providers.

Pathways for Better Health:

Provides non-medical case management services to support healthy outcomes for pregnant individuals and babies using a whole person approach to support and educate throughout pregnancy as well as the postpartum period.
