Research Request

The Southern Nevada Health District (SNHD) created an Institutional Review Committee (IRC) to review EXTERNAL requests for research with SNHD. The committee coordinates with the District Health Officer, Division Directors, and program managers. Ultimately the decision resides within the District Health Officer, the Division Director, or their designated representatives’ purview.

The IRC is responsible for reviewing all EXTERNAL requests for research with SNHD or using SNHD-owned data. Examples of such requests are client/patient surveys, program evaluations, extractions of data from any of SNHD information systems. The requests should be directed to the IRC Committee via the IRC request form.

The objective of the IRC is to review and determine the feasibility of every request submitted, to consider the extent to which each request aligns with the mission and purpose of SNHD, and to ensure that each request does not adversely impact SNHD’s operations. The IRC shall meet with the impacted division, program or staff before making a recommendation to SNHD leadership. The IRC shall coordinate and inform the appropriate division, program or staff that may be involved to ensure that there is a transparent and objective process regarding the implementation of the IRC’s recommendation.

Instructions: Please submit a written letter signed by the individual requesting the research. In addition,
please complete this IRC Request Form. Both the letter and the IRC form shall be submitted prior to beginning any review. You must allow the IRC a minimum of 30 days from the date of your completed submission to consider the request and provide a response. Please do not access any data unless you have received written approval from the IRC. Use of, or collection of, data is conditional upon IRC review and approval. Please submit both the letter and the completed IRC form and any attachments to the following email address:

This form is for research collaboration request. If you’re requesting for data, please use the public health data request portal.
