Private Records
Private records are records with personally identifiable information about a client. Private records can only be requested by the individual or the legal guardian of a child under 18 years of age.

Public Records
Public records include inspections of permitted facilities, statistics, meeting minutes, etc.
Board of Health Records - Complete a Board of Health Records Request Form PDF 109KB if you need a public record from the Board of Health meeting. Visit the Board of Health webpages for agendas and minutes from 1996 to present, supporting documentation from Sep. 2007 to present and audio recordings from Dec. 2010 to present.
General Public Records - Complete a Public Records Request Form if you need a public record, such as restaurant, pool or hotel inspections.
