U.S. Passport Application Acceptance Office

The U.S Passport Application Acceptance office is designated by the U.S. Department of State to accept passport applications.

Required Documents

Prior to your visit please complete and print a Passport Application Form (DS-11).
Do not sign the application form.

  • All applicants, including minors, must be present to process applications
  • Forms DS-11, completed but do not sign
  • Evidence of U.S. citizenship
  • Photocopy of Evidence of U.S. citizenship
  • Photo ID
  • Photocopy of Photo ID
  • Passport Photo

*Nevada Driver Authorization Cards with the notation “Not Valid for Identification” are not acceptable for passport applications.

Passport Types and Fees

Two separate payments are required.

  1. Payable to the Southern Nevada Health District for the application fee and photo (optional)

    Acceptable payment types: Visa, Mastercard, Discover, American Express, money order or, cashier’s check. NOTE: Cash and personal checks will not be accepted.

  2. Payable to U.S. Department of State – must be in the form of a check, money order, or cashier’s check. For help calculating Department of State fee’s see Passport Fee Calculator.

Passport Renewals

Passport renewals are completed by the passport holder by mail. If you have/had a U.S. passport you may renew your passport without appearing in person at a passport acceptance facility if your most recent passport:

  • Can be submitted with your application
  • Is undamaged (other than normal “wear and tear”)
  • Was issued when you were age 16 or older
  • Was issued within the last 15 years
  • Was issued in your current name (or you can document your name change with an original or certified copy of your marriage certificate, divorce decree, or court order)

If any of the above statements do not apply to you, you must Apply in Person using form DS-11.
