The Southern Nevada Health District are currently accepting appointments for Passport Services. Choose date and time below, then complete and submit the requested information. Please arrive at 280 S. Decatur Birth and Death Certificate Office at your selected registration time and check in with a staff member. If you need assistance with registration, please call 702-759-0904.

Below is a checklist of items/requirements needed for your appointment. All items must be checked to schedule an appointment.

• Form DS-11 is completed but not signed
• Evidence of U.S. citizenship
• Photo ID - if you do not have a valid NV DL please bring a second form of ID such as a Social Security Card
• Passport Photo
• Personal check or money order payable to the U.S. Department of State
  • $100.00 for the passport book if applicant is 15 or younger, $130.00 if 16 or older.
  • Include an additional $60.00 for expedited processing.
• Credit/Debit card or money order payable to SNHD for $35.00 execution fee.
• All applicants, including minors, will be present to process application(s). *If minor applicant is 16 or older only 1 parent must be present
