Monthly Statements & Vaccine Records

For your convenience, the health district will bill you on a monthly basis and mail a statement with the names of employees vaccinated, as well as the dates and types of vaccines administered.

Our electronic vaccination record-keeping is second to none.

• Employees receive shot records at the time of vaccination.
• Each vaccine administered is entered into our database by employee name, date of birth and company name.
• Vaccination records are entered into the state vaccination database.
