All Health Card applicants must provide valid identification. All identification documents must be originals, not photocopies.
Valid State-Issued Driver License
If you have a valid driver license from one of the states listed below, it will suffice for identification purposes. A non-driver DMV issued ID card may substitute for a driver license.

Other Identification Documents
If you do not have a license from one of the above states, you must furnish any two of the following identification documents, one of which must contain a photo:
- Driver license from another state or county (a non-driver DMV issued ID card may substitute for the driver license)
- Identification card issued by a foreign consulate (matricula consular)
- Social Security Card (a letter from Social Security may substitute – see below)
- Utility billing in your name and address (electricity, water, and gas)
- Local work ID (industry related)
- Bankcard ID (with photo)
- Sheriff’s Card
- Certified United States birth certificate
- Certified foreign birth certificate
- United States Passport
- Foreign Passport with Admissions stamp
- Certificate of Citizenship or Naturalization
- Consular report of birth abroad
- Permanent or temporary Resident Card
- Resident Alien Card or 1-551 Receipt
- Employment Authorization Card
- United States Military Identification or Military Report of Separation
- Local school ID with birth certificate (minors- under 18 years of age)
- Other valid ID equivalent to the above
