1. The Southern Nevada Health District travels to Mesquite once a month to process new, renewal and duplicate health cards. Download the Health Cards Classes PDF (19 KB/1 page) for the complete schedule.
   
   
2. Health cards are processed at the CasaBlanca Resort and Casino at 950 W. Mesquite Blvd. *Note: On Feb. 14, 2008 health cards will be processed at the Oasis Resort.
   • Check-in time for the first session is 8:15 a.m. to 9 a.m. Doors close and movie starts at 9 a.m.
   • Check-in time for the second session is 10 a.m. to 10:30 a.m. Doors close and movie starts at 10:30 a.m.
   • No one will be admitted once the doors close.
     
     
3. The fees are as follows:
   1. Food handler, massage therapy & health club attendant cards - $50; valid for 3 years.
   2. Child care, reflexology & aesthetician cards - $40; valid for 2 years.
   3. Adult group care cards - $45; valid for 2 years.
      
      An additional $15 late fee will be applied to card renewals that have expired.
      
      Barber and Body Art Health Cards are processed at the Ravenholt Public Health Center.
      
      
4. Please bring two forms of valid identification, one of which must contain a photo (see valid i.d. list).
   
   
5. When you arrive, you will fill out an application form, or you may print a PDF form in English PDF (55 KB/1 page) or Spanish PDF (63 KB/1 page), fill it out, and bring it with you accompanied with two (2) forms of valid identification (see valid i.d. list).
   
   
6. You will receive a Hepatitis A vaccination for a Food Handler card.
   • Please bring your immunization record with you.
   • Clients under the age of 18 must have a parent or legal guardian present or a letter signed by the parent or guardian, giving us permission to immunize the minor.
     
     
7. If you need a child care card, adult group care card, or other type of health card, you will receive a Hepatitis A vaccination and either a skin test or x-ray for Tuberculosis.
   • Please bring your immunization record with you.
   • Clients under the age of 18 must have a parent or legal guardian present or a letter signed by the parent or guardian, giving us permission to immunize the minor.
     
     
8. If you are unable to view the movie during the session, you have 30 days from the date of your application to do so. Failure to complete the one hour movie within 30 days of applying will result in a full-price reprocessing fee.
   
   The movie is in English during the first session and Spanish during the second session.
   
   
9. You will be given a 30 day temporary card to use until you view the movie.

Duplicate health cards are available to those who have a current Health Card. The fee is $20. Please bring two (2) forms of valid identification, one of which must contain a photo (see valid i.d. list).
