Foodhandler and Child Care
Health Card Requirements
 
  First Time Health Card Applicants

The fees are as follows:

  1. Food handler, massage therapy & health club attendant cards - $50; valid for 3 years.
  2. Child care, reflexology, aesthetician, barber & body art cards - $40; valid for 2 years.
  3. Adult group care cards - $45; valid for 2 years.

After payment, you will receive a Hepatitis A and/or TB skin test depending on the type of health card applied for, take a photograph and attend a movie presentation within 30 days.

You will receive a temporary health card which is valid for one month, and a receipt to turn in at the movie. Your health card will be mailed to you after you have attended the foodhandler class. If administered, TB skin test results must be read by staff.

Health Card Renewal Applicants (On Time)

The fees are as follows:

  1. Food handler, massage therapy & health club attendant cards - $50; valid for 3 years.
  2. Child care, reflexology, aesthetician, barber & body art cards - $40; valid for 2 years.
  3. Adult group care cards - $45; valid for 2 years.

After payment, you must receive a Hepatitis A and/or TB skin test depending on the type of health card applied for, take an updated photograph and attend a movie presentation.

Health Card Renewal Applicants (Expired)

An additional $15 late fee will be applied to card renewals that have expired.

NRS 446.030 “Foodhandler” Defined

“Foodhandler” means any person employed in or operating a food establishment, whether employer, employee or independent individual who handles, stores, transports, prepares, manufactures, serves or sells food, or who comes in contact with eating or cooking utensils or other equipment used in the handling, preparation, manufacture, service or sale of food.

Persons arriving late will not be admitted to the class.
Children are not allowed in the movie.
