Time and Location
Health cards are processed at the CasaBlanca Resort and Casino at 950 W. Mesquite Blvd.
- Check-in time for the first session is 8:15 a.m. to 9 a.m. Doors close and the one-hour movie in English starts at 9 a.m.
- Check-in time for the second session is 10 a.m. to 10:30 a.m. Doors close and the one-hour movie in Spanish starts at 10:30 a.m.
- No one will be admitted once the doors close.
Health Card Fees
- Food handler, massage therapy & health club attendant cards - $50; valid for 3 years.
- Child care, reflexology, and aesthetician cards - $40; valid for 2 years.
- Adult group care cards - $45; valid for 2 years.
- Duplicate health cards - $20; only available to those with a current health card. Bring two forms of valid identification, one of which must contain a photo (see valid i.d. list).
An additional $15 late fee will be applied to card renewals that have expired.
Cash only. Visa, Mastercard and personal checks are not accepted.
Note: Tattoo, permanent make-up and body piercing health cards are only processed at the Henderson Public Health Center. Barber health cards are only processed at the Ravenholt Public Health Center. Visit the Locations webpage for addresses and maps.
Valid Identification
All health card applicants must provide valid identification. All identification documents must be originals, not photocopies.
Bring two forms of valid identification, one of which must contain a photo (see valid i.d. list).
Health Card Application
When you arrive, you will fill out an application form, or you may print a PDF form in English PDF (26 KB/1 page) or Spanish PDF (26 KB/1 page), fill it out, and bring it with you accompanied with two forms of valid identification (see valid i.d. list).
Immunizations
Hepatitis A vaccination is required to receive a health card.
Tuberculosis screening is required to receive all types of health cards except the food handler health card. If you have a history of a positive TB test, bring the appropriate documentation.
Bring your immunization record with you.
Clients under the age of 18 must have a parent or legal guardian present or have a letter signed by the parent or guardian, giving us permission to immunize the minor and/or screen for TB.
Health Card Movie
If you are unable to view the movie during the session, you have 30 days from the date of your application to do so. You will receive a 30-day temporary health card to use until you view the movie. Failure to view the movie will require re-application and paying a full-price reprocessing fee.
The movie is in English during the first session and Spanish during the second session.
