Hepatitis A vaccination is required to receive a health card.

The vaccine is included in the health card fee and will be provided to you during the application process.

If you have already received one or two doses of the hepatitis A vaccine, bring your immunization record with you.

Parental Consent of Immunization

Non-custodial adults may accompany a child, however, written consent must be provided to the health district from the parent or guardian at the time of service. See the Parental Consent for Immunization of a Minor Policy for more details.
