You may complete the Application for a Health Card in person or you may download the form, fill it out, and bring it with you.
An application must be completed to get a new, renewal, reprocess or duplicate health card.
Grace Period
Due to the closure of the Ravenholt Public Health Center main building, a temporary grace period was instituted for health card renewals.
If your health card expired on or after April 16, 2012, you have 60 days to renew your card without being charged a late fee. If you received a temporary health card on or after April 16, 2012, you have 60 days instead of 30 days to see the food safety movie, if required. This policy will remain in effect until further notice.
Exceptions
Applications are only processed on Tuesdays and Thursdays at the Mesquite Public Health Center.
Barber applications are only processed at the East Las Vegas Public Health Center. Note: Notary service is not available at this time.
Tattoo, permanent make-up and body piercing health cards are only processed at the Henderson Public Health Center. Visit the Body Art Health Card Requirements webpage for detailed instructions that must be completed before you can apply for a health card.
