Duplicate health cards are only available to people with a current (non-expired) health card.

The cost of a duplicate health card is $20. Two forms of valid identification are required.

Steps to getting a duplicate health card

1. Go to one of the four public health centers that offer health cards.
2. Take a number and an application.
3. Complete the application while waiting for your number to be called. Circle duplicate on the application.
4. When your number is called, go to the station as directed.
5. The clerk will process your application, verify your identification and give you a copy of your application form.
6. Take the application form to the cashier and pay for your health card.
7. Go to the information desk to have your health card printed.
