Operator/Technician Requirement Checklist
(Initial Application for Health Card)
Per the Southern Nevada Health District Regulations Governing the Sanitation of Tattoo and Permanent Makeup Establishments, applicants must have a minimum of six months continuous training in a duly-licensed establishment in Nevada, or another state or country (section 3.3.4).
All health permit holders have been provided with procedures to follow for all operators and/or technicians who will be working in their establishments.
Regulations for tattoo/permanent makeup operators and body piercing
technicians in Clark County are quite stringent. In order to obtain
a health card, the operator or technician must meet the following requirements:
- Be free of communicable
diseases
- Be a minimum of eighteen years of age
- Have a minimum
of six months experience or training
- Obtained a score of at
least eighty percent on a written examination
- Be tested for
Tuberculosis (TB) by skin test or chest x-ray
- Be immunized against
Hepatitis A and B
- Possess current certification by the American
Red Cross in Preventing Disease Transmission
Health Card Instructions
- The following is required at the time of application:
- Written proof from a previous employer of at least six months experience or training.
- Written proof must contain a date, be on letterhead (if possible), specify month/year dates of experience and have the signature both printed and signed.
- Written proof from a Clark County Health Permitted Establishment stating the applicant will be working in his/her establishment.
- Written exam fee of $113 (non-refundable) payable by cash, money order, Visa and Mastercard.
- Applications must be completed in person at the Henderson Public Health Center at 520 E. Lake Mead, Henderson, Nevada.
- An appointment to take the written exam will be scheduled upon receipt of the application and exam fee.
- Testing is by appointment only. The written exam must be completed in two hours.
- Study material for the exam consists of brochures and the Regulations Governing the Sanitation of Tattoo and Permanent Makeup Establishments. The regulations are available free online or for $2 at the public health center.
- Results of the exam will be available immediately.
- Applicants who pass the exam can can apply for a health card.
- Additional fees for the health card will be required.

- Applicants who do not pass can retake the exam. The written exam fee of $113 is charged each time the exam is taken until a passing grade is received.
- Within thirty days of passing the written exam, all applicants must begin their Hepatitis B vaccination series (section 3.8) and take the required American Red Cross training (section 3.9.1)
