Apprentice Requirement Checklist
Per the Southern Nevada Health District Regulations Governing the Sanitation of Tattoo and Permanent Makeup Establishments, applicants must have a minimum of six months continuous training in a duly-licensed establishment in Nevada, or another state or country (section 3.3.4).
All applicants not meeting this requirement, must apprentice for a minimum of six months in a health permitted establishment. All health permit holders have been provided with the procedures to follow for all apprentices who will be working in their establishments.
A temporary health card will be issued to apprentices meeting the following requirements:
- The following is required at the time of application:
- Written proof from a Clark County health permitted establishment stating the apprentice will be working in his/her establishment.
- Written exam fee of $113 (non-refundable) payable in cash, money order, Visa and Mastercard.
- Applications must be completed in person in Wing 1 of the Ravenholt Public Health Center at 625 Shadow Lane, Las Vegas, Nevada.
- An appointment to take the written exam will be scheduled upon receipt of the application and exam fee.
- Testing begins promptly at 10 a.m. The written exam must be completed in two hours.
- Study material for the exam consists of brochures and the Regulations Governing the Sanitation of Tattoo and Permanent Makeup Establishments. The regulations are available free online or for $2 at the public health center.
- Results of the exam will be available immediately.
- Applicants who pass the exam can apply for a temporary health card.
- Additional fees for the health card will be required.

- Applicants who do not pass can retake the exam. The written exam fee of $113 is charged each time the exam is taken until a passing grade is received.
- Within 30 days of passing the written exam, all applicants must begin their Hepatitis B vaccination series (section 3.8) and take the required American Red Cross training (section 3.9.1)
- Upon completion of at least six months training (section 3.3.4), the applicant must return to the Southern Nevada Health District Environmental Health Division with the following:
- Written proof from the health permit holder stating the required training has been completed.
- Documentation of the Hepatitis B vaccination (series of three).
- Documentation of American Red Cross training.
- Permanent Health Cards will not be issued without written documentation from the health permitted establishment where applicant will be operating.
